Summary
The Clinical Success team is responsible for clinical leadership and strategic clinical quality oversight of Signify Health’s clinician network conducting in-home and virtual health evaluations. The team provides clinical support to the network, ensuring clinicians deliver high quality, high-value services that align with Signify Health’s policies, and service-level commitments to clients.
The Clinical Network Lead is responsible for providing clinical guidance, support, and engagement through outreach, including but not limited to 1:1 coaching, field observations, Q&A sessions, and webinars to the network. The Clinical Success team is dedicated to fostering a positive clinician experience and clinical excellence.
This role will report to the Senior Regional Clinical Manager, Clinical Success.
Roles and Responsibilities
Proactive clinical network engagement, support, targeted coaching and ongoing performance monitoring of Physicians, Nurse Practitioners, and Physician Assistants within a designated region.
Perform comprehensive, as well as, targeted review of clinical documentation in order to identify appropriate coaching opportunities with follow-up 1:1 coaching.
Perform in-person and virtual ride alongs with providers.
Provide urgent or emergent clinical support to field clinicians in real time.
Collaborate with multiple departments and internal stakeholders to address and ensure high-quality services and clinician experience.
Participate in staff meetings, conference calls, and other meetings as needed.
Attend training sessions to acquire/enhance skills related to programs offered.
Actively participate in committees, webinars, and other special projects.
Support regional and national company-sponsored events as needed. Ability to travel up to 25% of the time including ad hoc travel as needed.
Complete periodic in-home and virtual evaluations for Signify Health annually. Obtain additional state licenses as requested by Signify Health. Minimum travel and field work requirements subject to change depending on business needs.
Perform other incidental and related duties as assigned.
Basic Qualifications:
Education/Licensing Requirements:
Board-certified Adult, Family, or Geriatric Nurse Practitioner, Physician’s Assistant, Board Certified, or Physician (MD/DO)
State license (unencumbered) required; no history of prior license actions or board-ordered restrictions that would prevent obtaining licensure in additional states upon request
Current credentialing with Signify Health or ability to obtain credentialing within one month of hire
Reside and be licensed in one of the states in Signify Health’s South or East Region which includes the following: AL, AR, GA, FL, LA, MS, NC, SC, TN, DC, DE, NJ, MD, PA, VA, WV, CT, MA, ME, NH, NY, RI, VT
Ability to:
Adapt to rapidly changing business needs, effectively prioritizing internal and external business requests with urgent deadlines
Navigate difficult conversations with high level of empathy and professionalism
Obtain proficiency with the Signify Health in-home evaluation (IHE) application and the IHE process within 3 months of hire
To work Signify Health normal business hours of M-F 8:00 AM - 5:00 PM CST; additional hours required to meet project deadlines as needed. Available for occasional evening and weekend calls
Work at a desk with regular use of computer and headset in a dedicated, private work area free from distractions.
Lifting requirements of 20 pounds occasionally unassisted
Ability to travel by airplane with overnight hotel stays.
Driver’s license must be in good standing with the ability to travel by vehicle
Understanding of:
Health risk evaluations including HEDIS, stars, and risk-adjustment
Basic skills in MS Office; moderate skills in Google Suite; Power BI, Salesforce or Five-9 experience preferred
Experience with:
Minimum 2 years of experience conducting health risk evaluations, preferred
Minimum 2 years of clinical experience pertinent to the member population(s) in which oversight is provided

The typical pay range for this role is:
$78,331.00 - $224,952.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
