The Master’s Level Social Worker at Hope PACE serves as a key member of the interdisciplinary care team, providing comprehensive psychosocial assessments, counseling, advocacy, and care coordination for participants enrolled in the PACE program. This role focuses on supporting participants and their caregivers in achieving the highest possible level of independence, dignity, and quality of life within the community.
The Social Worker collaborates with medical, nursing, therapy, and support staff to develop and implement individualized care plans that address participants’ physical, emotional, and social needs. Responsibilities include ongoing evaluation of psychosocial well-being, crisis intervention, resource linkage, and coordination of community-based services. The Social Worker also assists in facilitating family meetings, providing supportive counseling, and advocating for participant rights and preferences.
This position requires strong clinical judgment, excellent communication skills, and a compassionate approach to working with older adults and their families. The Social Worker plays a vital role in ensuring that care is holistic, participant-centered, and consistent with Hope PACE’s mission of promoting independence and enhancing the quality of life for seniors in a supportive community environment.
Key Responsibilities:
• Responsible for the direct social work case management services to PACE participants. Provides social services support to include participant screening, case management, counseling, and referral.
• Participate in the Interdisciplinary Team’s initial assessments, care planning and ongoing re-evaluations of participant’s case.
• Maintain current written case management records, including periodic reassessments of program participants.
• Refer participants to appropriate community agencies or facilities, act as a liaison with such organizations and as advocate for participants.
• Consult with and advise staff members as to the relationship of social, emotional and cultural factors to health and medical care, and as to the availability of social services in the community.
Knowledge & Skills:
• Excellent communication; via phone, in person and email.
• Requires individual therapy skills geared to geriatric patients, analytical thinking and problem solving abilities.
• Attention to detail & ability to handle confidential information following HIPPA guidelines.
• Must be flexible to work in other locations if needed.
Required qualifications:
• Master’s degree in Social Work from an accredited school.
• Two years’ experience in a health related area desired.
• One year of elder care experience required.
• Must be licensure eligible: LMSW, LISW.
• Valid Florida Drivers’ License with good record
• Automobile Insurance
• Clear level 2 background check

$32.00 - $80,435.79
