About the Role
A home care company in Brooklyn is looking for a driven, people-oriented professional with intake experience to grow into a marketing and outreach role focused on generating Private Duty Nursing cases. You'll build meaningful relationships with hospitals, clinics, care managers, and social workers — becoming the face of the organization to the referral community while leveraging your intake background to support a seamless patient experience.

Key Responsibilities

Relationship Building & Outreach

• Establish and nurture relationships with hospitals, clinics, care managers, and social workers
• Actively market Private Duty Nursing services to grow the caseload
• Serve as the primary point of contact for referral sources

Intake & Operations

• Handle home care intake processes from initial inquiry through onboarding
• Ensure a smooth, professional experience for patients and referral partners
• Maintain clear communication across all touchpoints

Qualifications

• Prior home care intake experience required
• Strong phone and communication skills
• Comfortable in a relationship-driven, outreach-focused role
• Self-motivated and able to work independently
